Day Program Now Open! Our Adult Day Services Program is located at 280-B Gannett Drive, South Portland, ME 04106. We are open 5 days per week, Monday through Friday, from 9:00am to 3:30pm.We are specializing in serving the 45 year old age group and above with a mix of clients(Developmental and Intellectual Disabilities, Autism as well as clients with Dementia).Funding accepted through Section 21, Section 29 and Private Pay.Please contact 207-828-0048 for more information or to refer a client.
Elder Care Using a collaborative strengths-based approach, our team of clinicians offers outpatient therapy in the geriatric individual’s nursing facility or assisted living residence and in the home. Family therapy and behavior management support are also provided.
Shared Living Services - Section 21 and Section 29 This is a family or "roommate-like" support model where individuals with intellectual and developmental disabilities join a living arrangement which in some cases may include children and pets. An individual’s needs are matched with a Shared Living Provider, and are coordinated by a support team, case manager and DHHS. The Shared Living Provider supports the individual with living skills, personal care, organization and appointment schedules, transportation and community integration.
Achieving Independence in Maine - AIM AIM is a transitional housing program for young adults on the Autism Spectrum who are in their developmental "launching" years.
Community Support Program - 29 Waiver Direct Service Professionals (DSPs) work with individuals to increase community inclusion and develop social and community relationships. Art by Port Client.
Home Supports/Residential Services – Section 21 & 29 Waivers Direct Support Professionals (DSPs) work with individuals in agency managed residential group homes or in private homes throughout Southern Maine to improve and maintain their ability to live as independently as possible. Outpatient Therapy Children with intellectual disabilities and their families receive specific assessment and therapy treatment by licensed clinical staff. Specialized services include dual diagnosis of intellectual disability and mental health, positive support plans, trauma informed treatment, and social opportunities.Art by Port Staff.
Psychiatric Evaluation & Medication Management Port Resources offers comprehensive psychiatric assessments and on-going medication management for children and adults in the greater Portland area. Services include comprehensive evaluation, consultation, and ongoing medication management for children and adults with intellectual and developmental disabilities.
