Shared Living Option
We provide oversight to Shared Living Option homes throughout Cumberland and
York counties.
Our Shared Living Option (SLO) Program is the newest addition to the Port
Resources family of programs. Our SLO programs are spread from Falmouth to Gray
to Lebanon. These programs meet the needs of the individuals we support who are
more successful in a family style or “roommate-like” environment, sometimes
including children and pets. The individuals supported have a variety of needs
which are assessed by their support team so that they can be matched with a
Shared Living Option Provider.
The twenty-four hour care and support provided in these homes is outlined in
each individual’s Person
Centered Plan, which is created and monitored by Port
Resources. The level of care and support provided by the SLO Provider may
involve assistance with daily living skills, personal care, organization and
scheduling of appointments, transportation and community integration. Many of
the individuals living in our SLO Program attend day habilitation services
and/or have jobs in the community. Referrals for this program are sent regularly
from The Department of Health and Human Services in conjunction with case
management services.
The individuals interested in becoming SLO Providers are carefully screened,
including checks for criminal background, driving record, and references. Our
SLO Provider application asks thorough and detailed questions regarding
lifestyle, the physical home and experience working in the field. The
application is followed up by a detailed interview process. Also, all of our SLO
homes are thoroughly inspected by our Maintenance Coordinator
