In
Home Support
We offer an In-Home Support program for children in York and Cumberland
Counties.
Services are available to children aged
3-21 who have a diagnosis of Mental Retardation or Autism. In order to be
eligible for services, the child must be eligible for MaineCare.
Referrals to our agency are typically
made through the child’s case manager, but may also be made by the parent of the
child. These referrals may be made by contacting us at our Central office and
asking for the In-Home Support Department.
Once a referral is made, the family will
be contacted by our agency when we are able to provide staffing. An initial
meeting will be scheduled, at which time; an assessment of the child’s needs
will be conducted.
This assessment will help determine the amount of hours
that the child will be eligible to receive In-Home Support Services. After the
assessment is complete, a planning meeting will be conducted to ascertain the
goals and objectives the child and staff will be working on during the scheduled
hours of service.
The purpose of the individual treatment plan is to
assist the child in attaining new skills in the areas of behavior management,
independent living skills, and social skills. Progress towards these goals is
documented daily and compiled monthly. The Team re-convenes to review this
information on a quarterly and semi-annual basis, and re-work the entire plan
each year.
